Submit a Case

The chart below details which components are required when submitting a case for any of our products. Please send ALL required components, along with the appropriate order form. Be sure to pack fragile components like diagnostic wax-ups properly. Also, we strongly recommend submitting cases via UPS or FedEx.

In most cases, you can expect to receive an order acknowledgment email within a day of our receiving the case. Issues with improperly submitted cases or missing materials may cause delays in this process.

*Delays in design approval or requests for lab work like gingival or crown processing may require additional time. Cases submitted without required components may be subject to delays or additional lab work charges. We encourage waiting to schedule patients until after design approval, as that is when the final shipment date is locked in.
